Case Manager - follow-up and outcomes (Boston, MA)

Lumen Cobalt Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Boston, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
