Case Manager - follow-up and outcomes (Birmingham, AL)

Summit Lakeside Health Partners is a healthcare provider in Birmingham with clinics and support teams, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 48 people based in Birmingham. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Birmingham, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
