Case Manager - follow-up and outcomes (Baltimore, MD)

Clearpath Maple Housing manages affordable housing communities across Baltimore. We are hiring a Case Manager to take on follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance stores. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Baltimore. • Applications are reviewed on a rolling basis.
