Case Manager - follow-up and outcomes (Baltimore, MD)

Bluepeak Juniper Health runs clinics and patient services around Baltimore. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Baltimore, with some flexibility after the first few months. • We reply to every applicant.
