Case Manager - follow-up and outcomes (Baltimore, MD)

Cedar Fern Care Services is a social-care provider serving older adults and people with disabilities around Baltimore, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Full-time, on a regular day shift. • On site in Baltimore. • We reply to every applicant.
