Case Manager - follow-up and outcomes (Austin, TX)

We are growing our team at Summit Haven Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 174 people based in Austin. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for meal programmes. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Austin. • Applications are reviewed on a rolling basis.
