Case Manager - follow-up and outcomes (Atlanta, GA)

We are growing our team at Summit Basin Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. Benefits and stability attract long tenure. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for case files. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Atlanta. • We reply to every applicant.
