Case Manager - follow-up and outcomes (Anchorage, AK)

We are growing our team at Bluepeak Health Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 448 people based in Anchorage. What you will do • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Anchorage. • We will tell you what to expect at each step of the hiring process.
