Case Manager - follow-up and outcomes (Anchorage, AK)

Lumen Oak Health is looking for a Case Manager. The job centres on follow-up and outcomes for insurance authorisations, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Anchorage. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Anchorage. • Applications are reviewed on a rolling basis.
