Case Manager - follow-up and outcomes (Anchorage, AK)

We are growing our team at Amberfield Harbor Care Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Anchorage. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Anchorage. • Applications are reviewed on a rolling basis.
