Case Manager - follow-up and outcomes (Albuquerque, NM)

We are growing our team at Highbridge Quill Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Albuquerque. • We will tell you what to expect at each step of the hiring process.
