Case Manager - follow-up and outcomes (Albuquerque, NM)

Cedar Thistle Early Learning runs childcare centres in Albuquerque caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for meal programmes. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Albuquerque. • We reply to every applicant.
