Case Manager - follow-up and outcomes (Albuquerque, NM)

Redstone Vale Community Partners is a nonprofit in Albuquerque supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 47 people based in Albuquerque. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Albuquerque. • We reply to every applicant.
