Case Manager - follow-up and outcomes (Albuquerque, NM)

We are growing our team at Lumen Ember Early Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Albuquerque. • We will tell you what to expect at each step of the hiring process.
