Case Manager - follow-up and outcomes (Albuquerque, NM)

Cedar Cobalt Learning Group runs classrooms and tutoring across the Albuquerque area, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Albuquerque. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Albuquerque. • We reply to every applicant.
