Case Manager - follow-up and outcomes (Albuquerque, NM)

We are growing our team at Fernwood Sable Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 594 people based in Albuquerque. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Full-time, with a steady weekday schedule. • On site in Albuquerque. • We reply to every applicant.
