Case Manager - care planning (Wichita, KS)

Amberfield Mill Care Services Group provides in-home and community support in Wichita. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Wichita. • We will tell you what to expect at each step of the hiring process.
