Case Manager - care planning (Wichita, KS)

Nova Ember Community Partners is a Wichita charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Wichita, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
