Case Manager - care planning (Wichita, KS)

Clearpath Stone Civic Services works with county and city offices around Wichita, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Wichita. • We will tell you what to expect at each step of the hiring process.
