Case Manager - care planning (Wichita, KS)

We are growing our team at Silverline Fox Property Management and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease renewals. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Wichita. • We reply to every applicant.
