Case Manager - care planning (Virginia Beach, VA)

Westbrook Forge Civic Services is looking for a Case Manager. The job centres on care planning for public records, and it would suit someone who likes steady, practical work. You would join a large organisation of about 291 people based in Virginia Beach. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Virginia Beach. • Applications are reviewed on a rolling basis.
