Case Manager - care planning (Virginia Beach, VA)

Westbrook Lantern Learning Group is an education provider in Virginia Beach serving students and families. We are hiring a Case Manager to take on care planning. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
