Case Manager - care planning (Tulsa, OK)

Lumen Wilder Early Learning Partners is an early-learning provider in Tulsa. We are hiring a Case Manager to take on care planning. Children come first and so does looking after the teachers. What you will do • Help with applications and appointments for classroom supplies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Tulsa. • We reply to every applicant.
