Case Manager - care planning (Tulsa, OK)

We are growing our team at Nova Timber Early Learning Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Tulsa, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
