Case Manager - care planning (Tulsa, OK)

Summit Learning Group operates schools and learning programmes around Tulsa. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 104 people based in Tulsa. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Tulsa, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
