Case Manager - care planning (Tucson, AZ)

Fernwood Cedar Health provides primary and specialty care in Tucson, and we need a Case Manager to lead our care planning. You would join a small organisation of about 46 people based in Tucson. What you will do • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Tucson. • We will tell you what to expect at each step of the hiring process.
