Case Manager - care planning (Tucson, AZ)

Ironwood Meadow Learning Group is an education provider in Tucson serving students and families, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for student accounts. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with student accounts.
