Case Manager - care planning (Tucson, AZ)

Rivermark Drift Health Group runs clinics and patient services around Tucson. We are hiring a Case Manager to take on care planning. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Tucson. • Applications are reviewed on a rolling basis.
