Case Manager - care planning (Toledo, OH)

Ironwood Nest Community Partners Group is a nonprofit in Toledo supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people based in Toledo. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Toledo. • We will tell you what to expect at each step of the hiring process.
