Case Manager - care planning (Toledo, OH)

Westbrook Vale Housing is looking for a Case Manager. The job centres on care planning for resident services, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Toledo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
