Case Manager - care planning (Toledo, OH)

Northwind Umber Civic Services works with county and city offices around Toledo, and we need a Case Manager to lead our care planning. Benefits and stability attract long tenure. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for budget submissions. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Toledo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
