Case Manager - care planning (Toledo, OH)

Ironwood Corner Housing Group manages affordable housing communities across Toledo, and we need a Case Manager to lead our care planning. You would join a large organisation of about 956 people based in Toledo. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for lease compliance. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Toledo. • Applications are reviewed on a rolling basis.
