Case Manager - care planning (Toledo, OH)

Lumen Granite Housing is looking for a Case Manager. The job centres on care planning for lease compliance, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Toledo. • Applications are reviewed on a rolling basis.
