Case Manager - care planning (Toledo, OH)

Summit Sable Community Partners is looking for a Case Manager. The job centres on care planning for client intake, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Toledo. What you will do • Help with applications and appointments for client intake. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, on a regular day shift. • On site in Toledo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
