Case Manager - care planning (Tampa, FL)

We are growing our team at Bluepeak Stone Care Services Associates and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 17 people based in Tampa. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, Monday to Friday. • On site in Tampa, with some flexibility after the first few months. • We reply to every applicant.
