Case Manager - care planning (Tampa, FL)

We are growing our team at Harborlight Pine Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 54 people based in Tampa. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Tampa. • Applications are reviewed on a rolling basis.
