Case Manager - care planning (St. Louis, MO)

Northwind Haven Early Learning is an early-learning provider in St. Louis. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 26 people based in St. Louis. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for tuition and subsidies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with tuition and subsidies. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in St. Louis. • Applications are reviewed on a rolling basis.
