Case Manager - care planning (St. Louis, MO)

Bluepeak Thistle Community Partners runs community programmes across St. Louis, and we need a Case Manager to lead our care planning. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in St. Louis. • Applications are reviewed on a rolling basis.
