Case Manager - care planning (St. Louis, MO)

Summit North Learning Group is an education provider in St. Louis serving students and families, and we need a Case Manager to lead our care planning. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, Monday to Friday. • On site in St. Louis. • We will tell you what to expect at each step of the hiring process.
