Case Manager - care planning (St. Louis, MO)

We are growing our team at Fernwood Granite Property Management and adding a Case Manager whose main responsibility will be care planning. Maintenance and leasing staff share information every day. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident events. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in St. Louis. • We reply to every applicant.
