Case Manager - care planning (St. Louis, MO)

Harborlight Nest Community Partners runs community programmes across St. Louis, and we need a Case Manager to lead our care planning. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in St. Louis. • Applications are reviewed on a rolling basis.
