Case Manager - care planning (St. Louis, MO)

Nova Ember Community Partners runs community programmes across St. Louis. We are hiring a Case Manager to take on care planning. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in St. Louis. • We reply to every applicant.
