Case Manager - care planning (Spokane, WA)

Ironwood Birch Care Services Group supports families and individuals in Spokane with personal care and coordination. We are hiring a Case Manager to take on care planning. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, with a steady weekday schedule. • On site in Spokane, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
