Case Manager - care planning (Seattle, WA)

We are growing our team at Greenridge Vista Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Seattle. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Full-time, with a steady weekday schedule. • On site in Seattle. • We reply to every applicant.
