Case Manager - care planning (Savannah, GA)

We are growing our team at Rivermark Field Housing and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for resident services. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Savannah. • We will tell you what to expect at each step of the hiring process.
