Case Manager - care planning (Savannah, GA)

Fernwood Mill Care Services is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 43 people based in Savannah. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish.
