Case Manager - care planning (San Jose, CA)

Cedar Hollow Care Services is looking for a Case Manager. The job centres on care planning for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in San Jose. • We reply to every applicant.
