Nova Lakeside Health is looking for a Case Manager. The job centres on care planning for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of about 465 people based in San Jose. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for referrals. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries.
