Case Manager - care planning (San Jose, CA)

Summit Lakeside Health Group runs clinics and patient services around San Jose. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 66 people based in San Jose. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
