Case Manager - care planning (San Jose, CA)

Westbrook Nest Housing Group is a nonprofit housing provider in San Jose, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for resident services. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in San Jose. • We will tell you what to expect at each step of the hiring process.
